To request an initial evaluation please complete our Becoming a New Patient (Intake form – Word Doc version) or Becoming a New Patient (Intake form – PDF version)For children and adolescents this evaluation is two-part.  The first appointment is 30 minutes, the second is 45 minutes.  Adult evaluations are done in a single 30 minute visit.  Some evaluations may require additional visits, such as evaluations that involve testing for Autism or related disorders.

Once you are scheduled for an appointment, you will be given the password to the ‘Clients’ section where you will find forms in the ‘Step One: The First Visit’ section to fill out and bring with you to your appointment.

If you are using insurance, contact your insurance company to confirm that Dr. Park is covered on your insurance plan.  Your insurance plan is a contract between you, your employer, and the insurance company.  Your employer may contract with one insurance company to provide both medical and mental health services, or your employer may contract two separate insurance companies; one for medical coverage and another for mental/behavioral health care. Each insurance company contracts with professionals to provide specific services for their clients.  Not all services are covered by all insurance companies. 

• Note: You are responsible for obtaining any referral, authorization or precertification needed for your visit.  If you do not, you will be held personally responsible for the cost of your appointment.  Many insurance companies have different policies for accessing mental health providers than for visiting your primary care physician.  Your insurance company may have cut out-of-network benefits.  You can learn these by calling the insurance company.
